A charming community hospital the San Francisco East Bay is looking for an experienced Case Management RN to join their Care Coordination team. The RN Case Manager will report to the Nurse Manager Case Management and will work independently to deliver care coordination to patients in the acute care hospital setting.
The Registered Nurse Case Manager will work great day shift hours and will work with a multidisciplinary team of medical professionals. The Case Manager RN will have a minimum of 5 years of Case Management experience and will preferably have EPIC experience.
